A 15 years old boy presented with history of agricultural trauma to his right for one month. His vision was hands movements. Slit lamp examination showed a small self sealed corneal laceration, a large horizontal laceration in the anterior lens capsule with widely displaced rolled edges, traumatic cataract, and free cortical matter in the anterior chamber. Lens matter was aspirated with Simcoe Cannula through a 3.2 mm corneal slit incision, and a foldable intra ocular lens (IOL) was implanted in the bag through the laceration under local anesthesia. Next day his visual acuity was 6/5. At one week follow up an image was displayed in Retro Illumination Mode by Huvitz Auto Ref/ Keratometer Model HRK-7000 (Huvitz Co. Ltd, South Korea), showing IOL in the bag, a small self sealed corneal laceration (as small four dots at 10 o’clock), and a large horizontal laceration in the anterior lens capsule with widely displaced edges.
