In ambulatory patients, an oral macrolide (erythromycin, azithromycin, clarithromycin) or doxycycline is recommended for healthy adults. In hospitalized patients high dose penicillin, cefotaxime or ceftriaxone is a reasonable first choice with macrolide/fluorinated quinolone/doxycycline. Vancomycin may be required for possible high resistant pneumococcal strains and should be added to the cephalosporins in severely ill patients or those not responding to a beta-lactam.
