Abstract
Objective: To determine the types of infections in primary nephrotic syndrome. Study Design: case series. Place and Duration of Study: Pediatric Departments of Shaheed Mohtrama Benazir Bhutto Medical University at Ghulam Muhammad Mahar Medical College hospital Sukkur and Chandka Medical College hospital Larkana, from January 2009 to January 2011. Subjects and Methods: Hundred patients with infections in primary Nephrotic syndrome admitted in both departments were included in this study. The children of age 1-15 years who developed nephrotic syndrome for 1st time and fulfilling the International Study of Kidney Diseases of children criteria (ISKDC) (generalized edema, heavy proteinuria >40mg/m2/hr, hypoalbuminemia 250mg/dl) were enrolled for this study. A separate pro-forma was filled for each patient that includes pertinent clinical history and examination, base line investigations, especially to diagnose the case and to screen for the evidence of infections. Results: Out of 100 patients with infections in primary nephrotic syndrome 56% were male and 44% females, in majority of patient age were less than 5 years. Most common infections in this study were acute respiratory tract infection and urinary tract infection found in 47% and 25% respectively. The diarrhea was responsible for 12%, skin infection 5%, peritonitis 3% and other infections (sepsis, pulmonary tuberculosis, enteric fever, meningitis, chicken pox) 8%. Edema was found in all patients, fever in 86 patients, cough in 47 patients, dysuria in 20, vomiting in 13, diarrhea in 12, abdominal pain in 06, skin infection 05 patients, dehydration 03 and convulsions in 02 patients. Conclusion: Respiratory tract infection and urinary tract infection were the two common infections in primary nephrotic syndrome.
