Abstract

Background:Maxillaisperhapsthemostessentialandvisiblepartofthemid-face.Itisathree-dimensionalstructureandwhenreconstructingmaxillectomydefectstheprinciplesofaestheticsaswellasthebestfunctionaloutcomesaretakenintoaccount.TheaimofthisstudyistocomparetheAnterolateralThighFlap(ALTF)tothestandardoptionliketheRectusAbdominisFreeFlap(RAMFF)forthereconstructionofcomplexmaxillarydefects.Methods:ThisdescriptivecaseserieswasconductedattheDepartmentofPlasticandReconstructiveSurgery,ShifaInternationalHospitalIslamabad,Pakistanfrom2009to2016.Patientsofallagegroupswithcomplexmaxillectomydefects,(TypeIIIandIVaccordingtoCordeiroclassification)resultingfromtumourresection,trauma,osteoradionecrosisorinfection,underwentreconstructionwiththefreeanterolateralthighflapandtherectusabdominisfreeflap.Results:Overaperiodof8years,49RectusAbdominisfreeflapsand32AnterolateralthighfreeflapswereperformedforreconstructionofTypeIIIandIVmaxillectomydefects.Thefollowupwasweeklyfor1monthandthen3monthlyforthe1styear,6monthlyfor2ndyearandthenyearly.Allthepatientshadanuneventfulimmediaterecovery.Conclusion:ALTFhasadvantagesovertheRAMFFintermsofthedonorsitemorbidity,operativetimeandpostoperativerecoveryinthereconstructionofcomplexmaxillectomydefects.
